How it works

1

Add your practice once

Type your NPI and the details fill in from the NPI Registry. Add your Tax ID, license and signature. They are remembered in your browser.

2

Add the client and sessions

Enter the client and insurance details, search ICD-10 codes, and add each session with its CPT code, place of service, modifiers and fee.

3

Download the superbill

Check the warnings for anything insurers usually reject, then download a clean PDF to give your client.

What is a superbill?

A superbill is an itemized statement a provider gives a client who paid out of pocket. The client sends it to their insurance company, usually with the plan's own out-of-network claim form, to ask for reimbursement. It carries the same information an insurer needs from a claim: who provided the care, who received it, the diagnosis codes, and the procedure codes, dates and fees for each service.

Therapists, psychologists, counselors, dietitians, chiropractors, physical therapists and concierge or direct care physicians who do not bill insurance themselves use superbills so their clients can still use their out-of-network benefits.

What to include on a superbill

  • Provider: name and credentials, individual NPI (Type 1), group NPI (Type 2) if you bill as a group, Tax ID (EIN preferred over SSN), license number and state, and the physical practice address and phone.
  • Client: full legal name exactly as on the insurance card, date of birth, address, and the insurance company, member ID and group number. If someone else holds the policy, their name, date of birth and relationship.
  • Diagnosis: at least one ICD-10-CM code, with the primary diagnosis first, and each service linked to the diagnosis it treats.
  • Services: one line per session with the date of service, place of service code, CPT or HCPCS code, any modifiers, units and the charge.
  • Payment: total charges, the amount the client paid and the balance. Most insurers expect the balance to be zero.
  • Signature: the rendering provider's signature and the date.

Common CPT codes on therapy superbills

Type the code you documented for each session. The descriptions below are short summaries, not the official CPT wording.

CodeUsed for
90791Diagnostic evaluation (intake), no medical services
90832Individual psychotherapy, about 30 minutes (16 to 37)
90834Individual psychotherapy, about 45 minutes (38 to 52)
90837Individual psychotherapy, about 60 minutes (53 or more)
90846 / 90847Family psychotherapy, without or with the client present
90853Group psychotherapy
90839 / 90840Crisis psychotherapy, first 60 minutes and each extra 30

Place of service and telehealth modifiers

The place of service code describes where the client was, not where the provider was. Payer rules for telehealth change often, so check the plan before you send.

CodeMeaning
11Office visit
10Telehealth with the client at home
02Telehealth with the client somewhere other than home
95Modifier: live audio and video telehealth (some plans still ask for GT)
93Modifier: audio-only telehealth

Why superbills get rejected

  • Missing or wrong NPI, or no Tax ID.
  • A PO box instead of the physical address where care was given.
  • No diagnosis, or only a Z code, which most plans do not reimburse on its own.
  • A balance still showing as due.
  • A place of service or modifier that does not match how the session happened.
  • A session length that does not match the CPT code billed.
  • Sending it after the plan's filing deadline, which can be as short as 90 days.

Superbill Generator FAQ

Is this superbill generator free?

Yes. Make and download superbills without an account up to 3 times a day, or 5 times a day with a free account. Pro removes the daily limit.

Do I need an API key or account for the NPI lookup?

No. The lookup uses the public CMS NPI Registry. Only the NPI or the provider name you type is sent. Client details are never sent anywhere.

Where is client information stored?

Nowhere. The superbill is built in your browser. Your practice details and saved services are remembered on your own device so you do not retype them, and you can turn that off. Client details are cleared when you start the next superbill.

Can I include several sessions on one superbill?

Yes. Add as many service lines as you need. Most providers issue one superbill a month. Use Repeat weekly to copy a session every 7 days through the end of the statement period.

Does it include CPT code descriptions?

You type your own CPT or HCPCS codes and descriptions, and you can save the ones you use often. ICD-10-CM diagnosis codes and descriptions are built in because the list is public.

Can clients use a superbill for HSA or FSA reimbursement?

Often, yes. Many HSA and FSA administrators accept an itemized superbill as proof of a qualified medical expense. Clients should check their administrator's rules.

Does a superbill work for Medicare or Medicaid patients?

Usually not. Medicare and most Medicaid plans need the provider to file the claim, and enrolled providers generally cannot bill those patients privately for covered services.

Can I edit a superbill later?

Yes. Click Save to file to keep a copy of the form on your computer, then Open file to load it and make changes.
