A superbill is an itemized statement a provider gives a client who paid out of pocket. The client sends it to their insurance company, usually with the plan's own out-of-network claim form, to ask for reimbursement. It carries the same information an insurer needs from a claim: who provided the care, who received it, the diagnosis codes, and the procedure codes, dates and fees for each service.
Therapists, psychologists, counselors, dietitians, chiropractors, physical therapists and concierge or direct care physicians who do not bill insurance themselves use superbills so their clients can still use their out-of-network benefits.
Type the code you documented for each session. The descriptions below are short summaries, not the official CPT wording.
| Code | Used for |
|---|---|
| 90791 | Diagnostic evaluation (intake), no medical services |
| 90832 | Individual psychotherapy, about 30 minutes (16 to 37) |
| 90834 | Individual psychotherapy, about 45 minutes (38 to 52) |
| 90837 | Individual psychotherapy, about 60 minutes (53 or more) |
| 90846 / 90847 | Family psychotherapy, without or with the client present |
| 90853 | Group psychotherapy |
| 90839 / 90840 | Crisis psychotherapy, first 60 minutes and each extra 30 |
The place of service code describes where the client was, not where the provider was. Payer rules for telehealth change often, so check the plan before you send.
| Code | Meaning |
|---|---|
| 11 | Office visit |
| 10 | Telehealth with the client at home |
| 02 | Telehealth with the client somewhere other than home |
| 95 | Modifier: live audio and video telehealth (some plans still ask for GT) |
| 93 | Modifier: audio-only telehealth |

Often, yes. Many HSA and FSA administrators accept an itemized superbill as proof of a qualified medical expense. Clients should check their administrator's rules.
Usually not. Medicare and most Medicaid plans need the provider to file the claim, and enrolled providers generally cannot bill those patients privately for covered services.
