What to put on a medical fax cover sheet
- Sender and recipient: names, organizations, fax and phone numbers, so a misdirected fax can be returned.
- Date and page count: including the cover, so the receiver knows nothing is missing.
- Subject: enough to route it (for example "Prior authorization request"), without patient details on the cover.
- Confidentiality notice: that the fax may contain protected health information and what to do if it reached the wrong person.
