See all medical practice tools

Enter an NPI, a name, an organization, or a specialty with a city, ZIP or state.

How it works

1

Search or paste a list

Look up one provider by NPI, name, organization or specialty, or paste a list of NPIs and names.

2

We check the registry

Each NPI is validated and looked up in the public CMS NPI Registry. Names are matched with the state you give.

3

Download the results

Get a CSV with name, credential, specialty, taxonomy code, license, addresses, phone and fax for every row.

What is an NPI number?

A National Provider Identifier (NPI) is a unique 10-digit number that HIPAA requires on health insurance claims. Individual providers such as physicians, therapists and nurse practitioners get a Type 1 NPI. Organizations such as group practices, clinics and hospitals get a Type 2 NPI. The NPI never changes, even when a provider moves or changes specialty.

NPIs are issued by CMS through the National Plan and Provider Enumeration System (NPPES), and the registry is public. This tool searches that registry directly.

Who uses bulk NPI lookup

  • Credentialing teams: confirm provider details, taxonomy codes and license numbers before enrollment.
  • Billing staff: check referring and rendering NPIs before claims go out.
  • Recruiters and sales teams: turn a list of names into specialties, practice locations and phone numbers.
  • Practices: keep referral lists up to date with current addresses and fax numbers.

How an NPI is validated

The last digit of an NPI is a check digit calculated with the Luhn formula, using the prefix 80840. A number that fails the check is not a real NPI, so the tool flags it without searching.

NPI Lookup FAQ

Is NPI lookup free?

Yes. Single lookups are free. Bulk lookup includes 25 rows a day without an account and 200 with a free account. Pro includes 5,000 rows a day.

Where does the data come from?

From the public NPPES NPI Registry run by the Centers for Medicare and Medicaid Services (CMS). Results are the same as searching the registry directly.

Can I look up a provider by name?

Yes. Enter a first and last name and a state for the best match. In bulk mode, rows with several matches are marked so you can add a state or use the NPI.

What file formats can I upload?

CSV or plain text. From Excel, choose Save as and CSV. Columns can be named NPI, First name, Last name and State.

Why is an NPI not found?

It may have a typo, or the NPI may have been deactivated. Deactivated NPIs no longer appear in the registry.
